If you're a new patient looking to establish chiropractic care in our office, this appointment is for you! This first appointment is designed to allow time to do a thorough history and physical examination to find out what's going on and how we can help.
Please go to our website and fill out the new patient forms for chiropractic, and bring them with you to your appointment. You may also arrive 15 minutes early to fill out forms at our office. Please bring any insurance cards and ID. If you have any questions, please feel free to call.
A $45 deposit is required to hold your appointment time. The amount you owe may vary depending on the complexity of your case and your insurance coverage. The amount listed above is for the lowest complexity exam fee. Your account will be credited and you will be refunded any overage depending on your insurance coverage. However, if you do not cancel per our cancellation policy,your deposit will not be refunded. - Chiropractic-Re-examination $75
If you're an existing patient, and have had a new injury or illness, or something different is going on, this is the appointment for you. Please come in a few minutes early in case your insurance requires pre-authorization for visits. If you're not sure, please give us a call.

Massage and Bodywork- Bodywork 60 minutes $112
If you want to book a self-directed massage or other bodywork for an hour, this is the option for you. This option covers basic relaxation massage, reiki, polarity therapy, etc. Additional fees may apply for hot stone or other specialty treatments. (Be sure to check and make sure your provider offers the specific service you want).
If you have a medical necessity and will be using insurance, please use the 'Massage Therapy' option for booking, and call our office so we can verify your insurance before your appointment. If you're new, please go to the website and fill out the new patient paperwork and bring it to your first appointment, or arrive 15 minutes early to fill it out in the office. If you have any questions, please give us a call.

If you have a medical necessity and/or will be using insurance, please use this option for booking, and call our office so we can verify your insurance before your appointment. This service is for four units of medical massage, necessary to treat a specific injury or medical condition, ($35/unit).
If you're a new patient, please go to the website and fill out all the paperwork before your first visit, or come 15 minutes early. Please bring ID and your insurance information to your appointment. If you have any questions, please give us a call.
